Take a pumpkin
Find anything that tickles your fancy (if you know what i mean)
Copy an image of it
Stick it in the pumpkin
Enjoy

The pumpkin has transparency.
Put it's layer on top.
Done.

As mentioned, transparency is easy. Even if it wasn't transparent, you could always use magic wand, zero threshold, select white, delete.

Of course, if you want to insist on finding some difficult way to do it in which the pasted image is on the top layer, you could always paste in, select white area of pumpkin, invert selection, select paste layer, hit delete. Or something like that.

That's actually pretty well-done. One thing is that the 'echo' to the sides of the pumpkin is perhaps a bit overemphasised, but there's really nothing wrong with this that I can spot.

Yes, I would assume so too, but I've never seen anyone actually do that, and this is better than nothing.

To be clear, I've never seen anyone do that with Nepetasprite. Obviously even if I hadn't seen anyone do that with John, I just did.
